Fill Out New Patient Forms Online
After you schedule your appointment, we encourage you to fill out our new patient forms, so that you can submit it online at your convenience. Once submitted at the end of each form it will electronically download and sync into your patient chart. It’s that easy!

Get Directions
If you are unable to get directions on our Location page, you may call our office to get more detailed directions. We are located north of intersection Hwy 26/ Colleyville Blvd. and Tinker Rd. We are right next door to the Yard Art Patio furniture store and share the same parking lot in the red building.

What to Expect at Your First Visit
Smile for Miles Dental not only creates beautiful smiles, but we also welcome our patients with them too. During your first visit, you can expect to be welcomed by our highly trained and experienced team. Please have your ID and insurance card ready when checking in at the front desk.
Depending on your needs, you can expect to spend an hour with us on your first visit. During this time, we will take a full series of x-rays, provide an in-depth exam, and a comprehensive prophylaxis, if eligible. Dr. Hwang will address all of your concerns and recommend the best possible treatment to obtain your oral health goals.
